Plan A – Exclusive Provider Organization w/PCP - $1,750 Deductible
Plan B – Exclusive Provider Organization w/PCP - $2,250 Deductible
Plan C – Exclusive Provider Organization w/PCP - $3,750 Deductible
Plan D – CDHP Exclusive Provider Organization w/PCP - $3,500 Individual/$7,000 Family
Plan E – CDHP Exclusive Provider Organization w/PCP - $4,500 Individual/$9,000 Family
Plan F – CDHP Exclusive Provider Organization w/PCP - $6,950 Individual/$13,900 Family
