VADA currently offers Life insurance through UNUM and is mandatory for all employees who qualify for benefits. VADA offers the following four levels of term life insurance:

Converting or Porting UNUM Life Insurance

Upon termination of employment, becoming disabled or reducing hours, a former employee may want to either convert (change their group term life coverage to an individual whole life policy) OR port (take their group term life coverage with them and pay for it at group rates) their life insurance policy. This is very time sensitive and must be completed within 31 days of their coverage ending. Please visit the Forms & Guidelines page for copies of “Keeping your life insurance” brochure along with a sample letter, rates, and election forms to provide your employees upon eligibility.
