Frequently Asked Questions

1. How do I earn Wellness Incentives?

A. You earn one point for each of the 10 activities you complete. For example, you can get an on -site wellness screening offered at participating dealerships or get a checkup from your health care provider to earn one point. Even if your dealership doesn’t participate in the screenings, you can still satisfy this criteria by getting a checkup from your healthcare provider. If you earn 8 or more out of the 10 possible points you will be eligible for the Incentive Rewards. To be eligible for the Wellness Incentives, you must be enrolled in Blue Cross and Blue Shield of Vermont medical coverage for active employees.

2. How do I complete the “offline” criteria?

A. Some of the criteria can be complete d by participating in programs offered by your “Jump Start to Better Health” program. These programs include on-site wellness screenings a pedometer challenge and a weight loss challenge. Your dealership needs to participate for you to have access to the se free programs. If your dealership is not participating in these programs, please encourage your management to sign up. If you have certain risks (such as high blood pressure, high cholesterol, high blood sugar or are overweight) you can also participate in the FirstLine Therapy Therapeutic Lifestyle Change program. Alternatively, if your dealership is not participating in these programs or you prefer to satisfy the criteria another way, you can have your health care provider sign a Health Examination Form to certify that you have satisfied the criteria. If you are a non -smoker or have completed a smoking cessation program, you can fill out and submit a Non-smoker Certification Statement (or your health care provider can indicate this on the Health Examina tion Form).

3. Are there any other ways to satisfy these criteria?

A. You can also get your weight, blood pressure, cholesterol, and blood sugar checked at an on-site wellness screening. Any normal results will be automatically uploaded to your Wellness Incentive Tracker at your Blue Health Solutions online wellness center You can also contact Green Mountain Wellness Solutions, Inc. at (802) 229 -2038 or via mail at 61 Elm St., Montpelier, VT 05602 to request reasonable accommodations if a disability or medic al condition prevents you from being able to complete any of the criteria.

4. How do I find out what a normal blood pressure, weight, cholesterol, or blood sugar level is?

5. How and when will the “offline” criteria show up in the Wellness Incentive Tracker?

A. Your participation in any on -site wellness screening or challenge (pedometer or weight loss), as wel l as any normal results from an on-site wellness screening will be automatically uploaded to your Wellness Incentive Tracker at your Blue Health Solutions online wellness center within 2 -4 weeks of completion. Any criteria that you satisfy by submitting a Completion Form that has been filled out by your healthcare provider will also be uploaded to the site within 2-4 weeks of submission. This is also true for any tobacco-free forms that you submit.

6. Is there a deadline to complete the Health Assessment or other criteria?

A. The Health Assessment, any Online Health Coach Programs, and any Completion Forms should be completed and submitted by February 28, 2009 to earn incentives for the plan year.

7. What are the Incentive Rewards?

A. Upon completion of 8 or m ore criteria, each employee will receive a customized fleece jacket with the “Jump Start to Better Health” logo and a certificate of completion At the end of the program year on February 28, 2009, all employees that have satisfied at least 8 criteria will be entered into a random drawing for twenty $500.00 cash prizes. The total value of these cash prizes is $10,000.00.

8. What is an Online Health Coach program?

A. The Online Health Coach (OHC) at your Blue Health Solutions online wellness center provides m embers with programs designed to drive healthy lifestyle changes. The OHC blends wellness and disease management coaching and includes programs such as: Exercise, Nutrition, Weight Management, Smoking Cessation, Stress, Diabetes and Heart. Your individual risk factors, self -care and stage of change are addressed in a program tailored to your health risks and needs. A Low Back Pain and Asthma program will be offered in a future release of the site.

9. How do I complete an Online Health Coach program?

A. You b egin by either completing the Health Assessment at your Blue Health Solutions online wellness center or by completing a branching profiler that tailors the questions it asks according to your input. Upon completion of the questionnaire, you are given immed iate feedback to provide specific interventions based on health needs, risk and stage of change. Each program tracks your progress through various levels of the program. In order to advance in the programs, users must complete a series of tasks in each mod ule. The majority of the programs take about 5 weeks by completing one module per week. Incentive points are awarded at the completion of the program.

10. What if I don’t have a health care provider?

A. You can have your blood pressure, cholesterol, weight, and blood sugar checked for free .
at an on-site wellness screening.

11. How do I get an on -site wellness screening?

A. Wellness screenings are free, take about 20 minutes, and are offered once per year at every dealership that elects to participate. These scr eenings are typically scheduled from March through May. If your dealership doesn’t participate in the wellness screenings, you can still satisfy this criteria by getting a checkup from your health care provider.

12. How do I complete a pedometer challenge?

A. The annual pedometer challenge is a fun, free, and friendly group competition between dealerships that takes place each fall. If your dealership doesn’t participate in this challenge, you can still complete this criteria by complying with your health care provider’s physical activity plan.

13. How do I complete a weight loss challenge?

A. The weight loss challenge is new for 2008. It is a fun, free, and friendly group competition between dealerships that takes place in the spring. If your dealership doesn’t participate in this challenge, you can still satisfy this criteria by maintaining a normal weight OR by complying with your health care provider’s treatment plan OR by participating in the FirstLine Therapy program.

14. Where do I find out about smoking cessation programs?

15. How do I get credit for the items that require my health provider?

A. Print a copy of the Health Examination Form and take it to your health care provider. He/she can fill it out and submit it to our wellness vendor, Green Mountain Wellness Solutions.
